If you have received confirmation that your registration was selected for activation congratulations. You will soon be receiving a membership package via mail. But, that's just the beginning. 


In order to TAKE BACK CONTROL, you'll need to activate your membership through the online system. This includes setting up USER ID's and PASSWORDS.  You can then enter in your personal healthcare information so that all of your future physician consults will be able to review your history to help make more informed and complete consultations. You can  have the consult notes from each visit sent to your primary care provider to assist in even greater continuity of care.

 Here are your options:

1.  STEP-BY-STEP VIDEO TUTORIAL. Activate your account Online NOW  by following the directions contained in the Step-by-Step Video Tutorial. (Click the link above)

2. DIRECT SIGN UP via PHONE CALL.  Call 844-529-5780 and follow the simple prompts to speak with a Member Services Representative.  They'll ask you all of the questions and enter your information into the system for you.

3.  ASSISTED SIGN UP.   Call 877-824-0099 and arrange a time for a local representative to meet with you in-person to assist in the Activation process.
